Sample contract letter

Below is a sample of the letter send to all clients wishing to use Victim Support Wandsworth Counselling Service.  A verbal agreement is made between the two parties based on the contract.

"We would like to welcome you to Victim Support Wandsworth Counselling Service (VSWCS).

VSWCS offers a free, one to one counselling service provided by qualified and advanced level trainee counsellors. We provide up to six, one hour weekly sessions with the option to continue. Anything you discuss in your session will remain confidential within VSW. We only break confidentiality if for any reason we believe that you may be at risk to your self or others and wherever possible this would be with your consent.

Our counsellors work within the codes of ethics and practice of VSW and the guidelines for safe practice of the British Association of Counselling and Psychotherapy (BACP) of which we have organisational membership. Copies of these are available at the office.

For training purposes our counsellors may ask your permission to make an audio tape recording of a session. There is no obligation to agree and tapes are secured with complete confidentiality.

We ask that if you are unable to attend a session that you let the office know at least 24 hours in advance. If you miss a session, without contacting us, we will write to you and keep the next appointment open. If two appointments are missed without contact then we will assume that you no longer wish to continue using this service.

We wish to maintain and improve our service. We would be grateful if you would complete (you do not have to disclose you identity) and return the Evaluation Form on ending your counselling with us.
